5x8-Hour (08:00–17:00) Pay: $3,160 Gross/Week Start: ASAP Join a
dedicated team providing high-quality care to patients in the
comfort of their homes. We’re actively seeking a skilled and
compassionate Registered Nurse (RN) Case Manager to support our
growing Home Health program in Novato, CA. Key Requirements: -
Active California RN License — REQUIRED - BLS Certification —
REQUIRED - Minimum 2 Years of Recent Experience as a Home Health RN
Case Manager (Not Hospice) - Proficiency with OASIS Documentation —
REQUIRED - Strong clinical assessment, time management, and
communication skills - Reliable transportation for patient visits
Job Description: - Manage a case load of home health patients,
delivering holistic, patient-centered nursing care - Complete
accurate and timely OASIS assessments, including SOC, ROC,
Recertifications, and Discharges - Collaborate with physicians,
therapists, and other members of the interdisciplinary team -
Educate patients and families on clinical conditions and care plans
- Maintain compliance with all regulatory and agency standards
