experience in home health, not hospice Description: This position
is responsible for the overall management of patients, including
the provision of direct care in accordance with current standards
and regulations. The role plans, organizes, and directs all patient
care services for patients in the assigned caseload. It ensures the
optimal degree of quality care is maintained. The responsibilities
include coordinating the interdisciplinary team to implement the
established plan of patient care. The position is also responsible
for managing visit utilization and controlling expenses. Delivers
holistic and individualized care to all patients in the assigned
area. Develops, implements, and manages an interdisciplinary plan
of care that considers psychological, sociocultural, spiritual,
economic, and lifestyle factors. Fosters and maintains
collaborative relationships between patients, their families,
physicians, and other healthcare providers through effective
communication. Adheres to policies, industry standards, and best
practices to promote a quality patient experience. Engages in
continuous growth and development in professional nursing practice.
Job Accountabilities: Conducts patient and family assessments
encompassing physiological, spiritual, and emotional areas.
Collaborates with physicians and other members of the healthcare
team to interpret and adjust the patient care plan. Partners with
peers and other healthcare providers to streamline patient workflow
and improve outcomes. Provides competent, compassionate, and
cost-effective patient care in accordance with evidence-based
nursing practices. Maintains a safe and therapeutic environment for
patients and their families. Delegates tasks appropriately and
effectively to team members based on individual capability.
Communicates with patients and families in a clear and
collaborative manner, ensuring understanding of treatment plans.
Delivers safe and effective patient care in compliance with
established policies and regulations. Upholds the confidentiality
of patient protected health information. Monitors patient care
interventions, assesses therapies, and evaluates outcomes. Conducts
ongoing education for patients and their families to ensure optimal
care. Reassesses and documents each patient's response to the plan
of care. Reports safety or compliance concerns via appropriate
mechanisms. Documents treatment information accurately in the
electronic health record. Develops and implements transition
planning that includes the patient and family in care for
