Physical Therapist Location: Carmichael, CA Agency: Ambition 24Hours Pay: $4,117 per week Shift Information: Rotating - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Occupational Therapist Skilled Nursing/Rehab Carmichael, CA (Increased Rate) Role overview An Occupational Therapist is needed for a full time day shift contract at a skilled nursing/post acute rehab facility in Carmichael, CA, serving adult and geriatric residents. This 13 week replacement assignment offers an increased rate and focuses on short term rehab and long term care in a SNF setting. Assignment details Location: Skilled nursing/post acute rehab facility, Carmichael, CA 95608 Setting: Skilled Nursing Facility (SNF) / post acute and long term care Position type: Contract, full-time, non permanent Contract length: 13 weeks (02/23/26 05/23/26) Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal break Days: Sun, Mon, Tue, Wed, Thu, Fri, Sat (actual work days set by facility schedule) Guaranteed hours: 36 hours per week Gross Blended rate: $58 - $79 p/h Order reason: Replacement Key responsibilities Evaluate residents ability to perform ADLs, mobility, cognition, and safety within a SNF environment. Develop and implement individualized OT plans of care to improve self care, functional mobility, and independence. Provide OT interventions including ADL retraining, therapeutic exercise, neuromuscular re education, balance and coordination training, and adaptive equipment use. Collaborate with the interdisciplinary team (PT, SLP, nursing, social services, physicians) on care planning and discharge goals. Document evaluations, daily treatment notes, progress notes, and discharge summaries accurately and on time in the EMR. Educate residents and caregivers on home programs, compensatory strategies, and safe use of equipment. Required qualifications and skills Active California OT license (or eligibility prior to start). Graduate of an accredited Occupational Therapy program (Master s or Doctorate). Prior SNF or post acute experience preferred; 1 year ideal for a 13 week contract. Current BLS/CPR for healthcare providers. Strong clinical reasoning, time management, and documentation skills with the ability to manage a full caseload
