Come brighten our participants days with your cheery smile and helping hands! LifeLong Medical Care is looking for a Program Aide to work at our Marin Adult Day Health Care, located in Novato, CA. The Program Aide will provide personal care for participants in the day program, help lead activities and assist in the rehabilitation area for group and individual exercises as needed, and assist serving meals and nourishment. This is a part time, benefit eligible position, working 30 hours/week, Mon-Fri, 9 am - 3:30 pm.LifeLong Medical Care is a large, multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community.   • Provides person-centered care and assists participants in the adult day health program who are primarily living with dementia, mild cognitive impairment, developmental disability or acquired traumatic brain injury.
  • Assists participants with activities of choice, personal care including toileting, changing, and transferring and serving meals and nourishment.
  • Communicates and interact in a professional team-oriented atmosphere with participants, families, caregivers, volunteers, fellow team members.
  • Monitors and reports behavior of participants (disposition, falls, wandering) to activity coordinator, RN, and social worker. Informs the RN of clinical changes immediately. Reads and records information in the appropriate record keeping system.
  • Sets up the room for daily program. Maintains neatness and organization in rooms. Monitors supply inventory.
  • In addition to the duties listed above, the Program Aide will be expected to work in other departments as needed. These duties may include activities and rehabilitation program (OT and PT).
  • As requested, assists with and leads activity programs, including group activities, 1:1 activities and socialization, special events, parties, shows, outside breaks, etc. and other duties as assigned.
    • Commitment to the provision of person-centered primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
    • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
    • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
    • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
    • Excellent interpersonal, verbal, and written skills.
    • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
    • Ability to see how ones work intersects with that of other departments of LifeLong Medical Care and that of other partner organizations.
    • Make appropriate use of knowledge/ expertise/ connections of other staff.
    • Be creative and mature with a can do, proactive attitude and an ability to continuously scan the environment, identifying and taking advantage of opportunities for improvement.Job Requirements
      • High school diploma or GED.
      • Requires the ability to lift/carry up to 50 pounds and push or pull objects exceeding 150 pounds, using appropriate body mechanics. Must be able to transfer participants needing maximum assist of one.
      • Requires the ability to stand, walk, push, bend, kneel and reach at arms length and overhead for prolonged periods of time.
      • Current CPR and First aid cards.
      • Fluent in English.Job Preferences
        • Certified Nursing Assistant.
        • One year experience working with frail seniors, adults with disabilities, and dementia patients.
        • Bilingual Spanish/English.
